Are you eligible to use Form DS‑82?
You can renew by mail using DS‑82 if all of the following are true:
- •You can submit your most recent U.S. passport book and/or card with this application (undamaged, not reported lost or stolen).
- •Your most recent passport was issued when you were age 16 or older.
- •Your most recent passport was issued less than 15 years ago.
- •Your passport has not been damaged, mutilated, or reported lost or stolen.
- •Your passport was not limited in validity due to damage, multiple losses, or non‑compliance with 22 C.F.R. 51.41.
- •Your name is the same as on your most recent passport — or it has changed by marriage or court order and you can submit certified documentation.
If any of the above is not true, you cannot use DS‑82. You must apply in person using Form DS‑11 instead.

Step 3 — Gather your supporting documents
Assemble the following items to include in your mailing envelope alongside your printed DS‑82.
One recent passport photo
- •Attach one (1) recent color photograph taken within the last 6 months.
- •Photo must be 2″ × 2″, printed on photo paper, with a white or off‑white background and a neutral expression. Glasses must be removed.
- •Staple the photo to page 1 of the form with four staples in the corners of the photo box — do not cover the face.

Your most recent passport
- •You must include your most recent U.S. passport book and/or card in the envelope.
- •It must be undamaged and issued less than 15 years ago.
- •Your old passport will be returned to you, usually in a separate mailing from your new passport.
Name change documentation (if applicable)
- •If your name has changed since your last passport was issued, submit a certified copy of your marriage certificate, divorce decree, or court order.
- •Photocopies are not accepted. You must send the original or a certified copy. All original documents will be returned to you by mail.
- •If you cannot document the name change this way, you must apply using Form DS‑11 instead.
Passport fees
There is no execution fee for DS‑82 renewals. Pay the application fee by check or money order payable to "U.S. Department of State."

Step 4 — Mail your application
Mail your completed DS‑82, photo, previous passport, fees, and any name change documents to the address that matches your state of residence.

Mailing tips
- •Use a trackable delivery service (e.g., USPS Priority Mail) when mailing your application.
- •Do not fold your form or photo. Use a large envelope (Tyvek or similar) to keep documents flat.
- •Do not send cash. Pay the application fee by personal check or money order payable to "U.S. Department of State."
- •Write the applicant's full name and date of birth on the check or money order.
After you apply
Track your application status online at passportstatus.state.gov. Your new passport and your old (cancelled) passport will typically arrive in separate mailings. Original supporting documents (marriage certificates, court orders) will also be returned.
