Step 3 - Compile Supporting Documents
Police report (recommended)

Passport fees (if replacing)
- •Report only: there is no fee to submit Form DS‑64 if you are only reporting the loss.

Proof of identity (if replacing)

Name change documents (if applicable)
- •If your name has changed since your lost passport was issued, and you are applying for a replacement, you must submit a certified marriage certificate or court order.
- •Photocopies are not accepted for name change evidence.

Option 1: Reporting only
Mail your signed DS‑64 along with a front and back photocopy of a valid government‑issued photo ID (such as a driver's license) to the Consular Lost and Stolen Passport Unit:

Option 2: Reporting and replacing
You must submit Form DS‑64 in person along with your new application (Form DS‑11) at an authorized acceptance facility (Post Office, Clerk of Court, etc.).
If you find your passport after reporting it
A reported passport is permanently cancelled and cannot be reinstated. Anyone travelling on a cancelled passport may be detained upon entering the United States. If you find the passport, mail it to the Consular Lost and Stolen Passport Unit (CLASP) at the address above for destruction.
Repeat loss warning: if more than one valid U.S. passport previously issued to you has been lost or stolen, your replacement passport may be limited in validity.
